How Medicare IRMAA works
Most people enrolled in Part B pay the standard premium. Higher-income beneficiaries pay an Income-Related Monthly Adjustment Amount for Part B and Part D. IRMAA uses modified adjusted gross income—adjusted gross income plus tax-exempt interest—from a prior federal tax return, generally two years earlier.
The 2026 standard Part B premium is $202.90 per month. This calculator applies the official 2026 filing-status thresholds and adds an optional Part D plan premium and Medigap or Medicare Advantage premium. It does not estimate deductibles, copays, coinsurance, late-enrollment penalties, or medical spending.
Planning Medicare with retirement income
Taxable withdrawals from a 401(k) or Traditional IRA, Roth conversions, capital gains, dividends, interest, pensions, and other income can affect MAGI and future IRMAA. Qualified Roth IRA withdrawals generally do not increase MAGI. Coordinate investment sales and account withdrawals across tax years rather than treating Medicare as an isolated premium.
